The Transitions Program
The program provides practical tools to help you identify your lifestyle patterns to initiate healthy changes through educational information, goal setting standards, means of tracking your progress, motivational information and more. Weekly communication and support will be provided through:
- concepts to address various aspects of well-being
- written material to support the information presented
- weekly reviews of previous observations
After you’ve completed the six-week program, you will have the exclusive opportunity to participate in more detailed classes on specific topics.
